KLM Laboratories Inc. built the KLM Digital Casting app as a Free app. This SERVICE is provided by KLM Laboratories Inc. at no cost and is intended for use as is.
This page is used to inform visitors regarding our policies with the collection, use, and disclosure of Personal Information if anyone decided to use our Service.
If you choose to use our Service, then you agree to the collection and use of information in relation to this policy. INFORMATION COLLECTION & USE:
For a better experience, while using our Service, we may require you to provide us with certain personally identifiable information, including but not limited to first name, last name, gender, birth date, weight, shoe size, 3D anatomical images, and photographs. The information that we request will be retained by us and used as described in this privacy policy. The personal information collected by the app may be your personal information, or that of your patient, customer or client. It is the user’s responsibility to inform their patient, customer or client of the privacy policy before submitting their information.
Personal information is collected by requesting it in the “Patient Details” form. The data is used by KLM Labs to fulfill prescriptions for custom foot and ankle foot orthoses. We may also use this information to help us create, develop, deliver, protect, and improve our products, services, content, and customer communications. We may also use personal information for internal purposes such as auditing, data analysis, and research to improve our products, services, and customer communications. This information is only shared with companies that have good privacy practices under the accordance of HIPPA regulations.
Personal information collected by the application is stored by the application as long as the application is installed on a specific iOS device. Once the data is uploaded to KLM Lab’s internal server it is stored by KLM Labs indefinitely. Being that data is transferred from the device with the application installed to KLM’s internal server for processing, this Service can be defined as online. The application also has links to KLM’s website, klmlabstore.com and online ordering system elab.klmlabs.com, making it possible for the user to navigate outside of the application by engaging a button or link within the application. It is the user’s responsibility to understand when they have navigated outside the KLM app for any reason. This policy statement only covers the information and activities occurring within KLM’s Digital Casting app. Please be informed of the policies of any other websites or applications you may navigate to outside of KLM’s app.
